Summary:
As a Social Work Associate at MU Healthcare, you will play a pivotal role in providing comprehensive support to patients and their families through the application of case work methodologies.
Collaborating closely with our master's level staff and under the guidance of our dedicated department manager, you will focus on evaluation, resource referral, counseling, and discharge planning to ensure the utmost care and well-being of our patients.
Detailed Description:
Engage in meaningful consultations and foster strong collaborations with healthcare providers and fellow staff members to create cohesive plans addressing the unique needs of our patients, thus delivering exceptional social services.
Exhibit exceptional empathy and understanding to determine our clients' precise requirements for services, and proficiently connect patients and families to vital resources, including transportation, lodging, and food, to alleviate their burdens during challenging times.
Meticulously document all social services provided, adhering strictly to departmental and hospital policies, and uphold precise departmental statistical records, allowing us to continually enhance the quality of care we provide.
Become a crucial architect of smooth patient transitions from the hospital by skillfully implementing discharge planning in close coordination with patients and their families, ensuring their safety and comfort.
Conduct thorough interviews with patients and their families to gather essential social information for healthcare planning, enabling the establishment of personalized treatment programs tailored to individual needs.
Additionally, offer compassionate counseling and support to patients and families in situations of limited complexity, helping them adapt to illness or disability.
Required
Qualifications:
A bachelor's degree in social work from a college or university accredited by the National Council of Social Work is required.
Experience in medical social work and/or current enrollment in the master's program in the medical sequence is preferred.
